Application form for any procedure through the Office of the Secretary:
Representation
When the person concerned can not carry out a procedure in the office of the secretary (except collecting the certificate) can authorize a third party, who must submit:
- Representation form.
- ID copy of the represented person.
- Identification of the person who carries out the procedure.
