Emotional disorders, such as depression, anxiety, and somatisation, are the most common mental health conditions among the general population, representing an economic cost of nearly ¿46 billion per year in Spain. Despite the substantial body of evidence supporting the effectiveness of psychological therapies for these disorders, they are still not the treatment of choice in Primary Care in Spain. Instead, psychotropic medications (antidepressants and anxiolytics) are often overprescribed, both in the short and long term.
Fortunately, significant progress has been made in incorporating psychologists into primary care in many autonomous communities, thanks to the PsicAP project (Psychology in Primary Care), one of the most important initiatives in mental health within this clinical setting. The PsicAP Project has demonstrated the effectiveness of transdiagnostic psychological treatments for emotional disorders in Primary Care in Spain, through a randomised multicentre clinical trial involving a sample of 1,700 assessed patients, with over 1,000 included, more than 300 treated in both groups, and 12-month follow-up data available for nearly 200 patients per group.

Fortunately, significant progress has been made in incorporating psychologists into primary care in many autonomous communities, thanks to the PsicAP project (Psychology in Primary Care), one of the most important initiatives in mental health within this clinical setting. The PsicAP Project has demonstrated the effectiveness of transdiagnostic psychological treatments for emotional disorders in Primary Care in Spain, through a randomised multicentre clinical trial involving a sample of 1,700 assessed patients, with over 1,000 included, more than 300 treated in both groups, and 12-month follow-up data available for nearly 200 patients per group.
The transdiagnostic psychological treatment significantly reduced emotional symptoms, with medium to large effect sizes at post-treatment assessment, and these results were sustained over time. Patients in the experimental group also showed significant improvements in secondary outcome measures, with small to medium effect sizes for functioning and various quality-of-life domains. Furthermore, large between-subject effect sizes were observed in reliable recovery rates, which were comparable to those achieved in the IAPT programme in the United Kingdom.
However, despite the strong momentum of the PsicAP initiative, it is now essential to assess whether the results obtained under ideal conditions hold true in real-world practice, and to evaluate various mechanisms of change within transdiagnostic therapy that influence treatment outcomes. For this reason, this research group aims to deliver psychological therapy in Primary Care centres in the Comunitat Valenciana.
